Overview
Occasionally, a note needs to have a data point edited or corrected after the release. Follow the steps below to learn how to amend a released note, without jeopardizing the integrity of the note.
Required Permissions and Prerequisites

- Group Notes
Step-by-Step Instructions
Step 1: Navigate to the Client's Chart
Navigate to the client chart by clicking on Clients in the top menu.
Step 2: Open the Notes tab
Click on the Select Client Data Tab at the top right.
Step 3: View the Note to Create the Addendum
After selecting View Note to the left of the appropriate note, locate the Addendums tab under the Tab Menu. Please note; this menu only appears on notes that have been released. If you do not see it in the dropdown of the Tabs Menu, the note is not entirely released.
Step 4: Create the Addendum
On the Addendum menu, click + Add New to enter the appropriate information. You must put a brief explanation as to what you are changing (or why) in the first field. Once you have added that, you may make your changes to the note by selecting a different date of service, service time, billing code or duration. Click Save in the bottom right corner.
