Overview
A disclosure refers to the release, transfer, provision of access to, or sharing of information outside the entity that holds it, such as a medical practice or institution. PIMSY provides tools to track any disclosures of Protected Health Information (PHI) made by your practice or organization, helping ensure compliance and accountability.

- Please assign the Create, Modify, View, and Delete permissions for the following rule(s):
- Client Disclosure
- Disclosure Reason…(Pick List)
- Disclosure Type…(Pick List)
Step-by-Step Instructions
Step 1: Setting Up the Disclosure Type in Pick Lists
You must first set up the Disclosure Type, Reason, and Delivery Types in Pick List Management. Go to:
Administration > Pick List Management
In the search bar, type “disclosure” and select Disclosure Types from the results. After verifying the setup of this list, click Disclosure Reason to review and confirm that list as well.

Step 2: Documenting a Disclosure
From the top navigation menu, go to:
Clients > Client Management
Select a client from the list to open their chart. From within the chart, click the Contacts tab, where the Disclosure List can be found at the bottom.
To create a new disclosure, click the blank row at the bottom and select a Disclosure Reason, Delivery Type, and Delivery Type. Fill out all other applicable columns and click Save at the top.
Step 3: Documenting a Disclosure
For reporting and tracking purposes, a global Disclosures list is available under:
Clients > Disclosures
This list includes search and filter options for any disclosure fields, making it especially useful for tracking outstanding balances related to records, or frequent requests.
