Overview
PIMSY has a customizable form-building tool available to all companies by utilizing our Q&A (questions and answers) module. This allows you to create your own customized forms throughout the system for providers and clients in the notes, Treatment Plans, Audits, Surveys, and many other areas to fit the needs of your practice. This article shows you how to take your Q&A Picklists, and build them into seamless forms to be used throughout the system by clinicians and clients.

You can customize the name of each form via the field labeled Your Name on the form set up. The profile rule will reflect that name rather than the generic title. For the purpose of these instructions, we have used the Mental Activity Pick List and will be building the form "Severity Measure for Specific Phobia — Adult"
Step-by-Step Instructions
Step 1: Navigating to Q&A Question Set Up
From the top navigation menu, go to:
Administration > Q&A Setup > Q&A Question Setup
If you are using PIMSY Platinum, you have the ability to add as many categories as needed in the following areas:
- Client Audits which display in the clients chart on a tab that can be relabeled via setting -217.
- Misc Company Q & A
Selecting a form will allow you to view and modify that form on the right side panel. To create a new form, select a default in one of the above categories and click the New button at the top. Type the new category name in the field labeled Your Name and then save.
*NOTE: Best practice, after creating a new form, exit the Q&A Question Set Up, refresh layouts and picklists, and then return to the Q&A Question Set Up. Now you can begin making edits to the form.
Step 2: Building Your Forms
First, let's number our available spots for questions. Go through and label the Order column in increments of 5, for as many questions as you anticipate. The numbering order tells PIMSY which order to display the questions in. Prior to the initial save, you must also have a Control.
- Enter your question as it should appear on the form in the Question field.
- Using the control labeled Section Header is a great way to add text to your form that does not have any action items needed.
- Assign the appropriate control. PIMSY comes with a variety of controls built in, such as Yes/No, Free Text space to insert tokens, Pick lists, Dates, and Scales.
- If you would like to create a box for the user or client to enter remarks, check the Remarks button.
- There are Pick List Check Box and Pick List Radio Buttons. If you want a user the freedom to check multiple answers for the question, you would select Pick list Check Box, If they should only be allowed one answer then you should assign the Pick List Radio Button control.
- If using the Pick List Controls, you will assign the Pick List Type that holds the answers you created from the first article. For our example we are using Mental Activity type.

Enter your question as it should appear on the form in the Question field.
You can repeat the steps to build as many questions or text fields as necessary.
If the question is required, check the Required Always check box.
Creating a form complete option at the end is a great way to make sure that all questions are answered. If the Forms Complete Question is checked to the far right of the entry, the system will prompt users and clients if there are any sections that are missing a required answer.
Step 3: Scoring Forms
You may have numerous scores that sum automatically. To do this, group the sections of questions by assigning them to the appropriate Include in Score column.
For example, this form will take all the questions that are checked here to Include In Score 1, refer to the number given to each of the answers in the Pick List Type, and automatically sum them onto the form.
Save prior to exiting.
Step 4: Assigning Forms
If the forms should be displayed on the client portal for the clients to complete, you will need to check Exposed On Client Portal.
Make sure any user that needs access to the form has the specific profile rules. The profile rules are renamed once you update Your Value of the Naming field on the form. Simply search for the text in the profile management to locate the appropriate form name, and double click to send it to the Assigned Rules for all applicable profiles.
